Introduction

In recent years, the international debate has focused around the crisis of health and human service systems (DHHS) decentralization in United States. It also emphasis on aspects related to the crisis of funding, which have been the rationale for reforms that introduced changes in management methods and mechanisms of financing, usually accompanied by proposals for restrictions on access in provision of services. Many of these proposals and arguments are present in the national contemporary ways presented as inexorable or inevitable solutions. To allow a clear position on these measures rationalizing is considered as the solution that make it necessary to know a little better about the content and situate them in the political and social contexts in which they emerged.

Discussion

Decentralization when see in comprehensiveness of care-priority prevention, and community participation are the main guidelines of the DHHS and the health surveillance. The issues which need to be addressed are the role of the federal government that comes down almost to the functions of financing and regulation, under the responsibility of the Department of Health and Human Services, DHHS. Only the actions related to control health of immigrants, regulation marketable drugs, health care for indigenous populations, production of statistical information, abortion and other epidemiological studies that are carried out by federal agencies. Most state functions are performed by the states through specific agencies that operate under federal oversight and coordination (Miles, 1974). Financed in large proportion with federal funds, this department is responsible for communicable disease control, environmental health, maternal attention, child preventive, provision of services to the poor and other services.

It is the responsibility of private health insurance through employers, who are responsible for most of the prize to the coverage of the majority of the population with regard to hospital care. Government services are restricted to the poor and elderly indigent and the elderly patients over 65 years and some population with disabilities (Miles, 1974). The Medicaid absorbs most of the public funds allocated to health and has Tripartite funding, where the federal government comes in with about 50% of expenditures and state and local governments with the other half. Sources are tax basically income tax in the case of federal taxes on consumption and property and in the case of state and local resources.
