Introduction

In this particular assessment item, I am selecting one of the case studies provided by the instructor. I will discuss the care and management of that patient/client. Here, I am considering myself a nurse, caring for the client that I have chosen for case study. Here, I will address the issues of,

Client assessment, i.e. to provide an overview of the patient/client, to describe the phenomena and tools. I will conduct a comprehensive assessment of the client's wound(s) or pain, the client's current cultural, physical, psychological and social needs. It is followed by the discussion on Patho-physiology & contributing factors present on the literature, used to identify and discuss the patho-physiology and contributing factors for the development of the client's wound(s) or pain, Client care goals or Outline goals for the care of this client in order of priority. Nursing interventions drawing on evidence based wound or pain management practice, critically analyzing the range of nursing interventions that could be implemented to manage the client's wound(s) or pain, Providing clear rationale (reasons) for your chosen interventions. Moreover, I am demonstrating critical analysis on nursing interventions and wound/pain management practices outlined in the literature, and identifying which specific nursing interventions would be better for this client. Additionally, I am demonstrating a “Discharge planning Formulate” and Reflection on Learning.

Overview of Patient

His name is Mr. George Pramana. His age is 24 years. He is related to the Occupation of Fitter and turner. Currently he is living with his two friends, in a shared and rented flat. His Leisure activity is enjoying fishing. Ethnically, he is Indonesian. He lived in Australia since 8yrs of age, as his family live interstate. His comorbidites status is Nil. Regarding his health history, he is an Asthma patient from his childhood. Once as a child, he had fractured his right humerus when he was 9 years of age. Reason for his admission is that, five days earlier, George was renovating his classic car. Unfortunately, the welding torch ignited some of the fluid on the ground and George. As a result, he sustained full-thickness burns to his calves on both legs and deep-partial thickness burns to his thighs on both legs. His feet were protected by shoes, so were not burned. He dropped to the ground and rolled over the garage floor to extinguish the fire. He then took off his clothes and stood in the drum of rainwater. That drum had been standing outside the garage for months. Then the ambulance arrived. Four days ago he was assessed in the burns unit. His left calf and ankle was diagnosed to have circumferential burn injury. He was taken to the operating room for an escharotomy of his left lower limb. Today I have notice that there is a distinctive and unpleasant smell coming from his burn sites and the dressings are soaked through with thick, greenish fluid. George also tells me, that the dressings feel tight and uncomfortable. His observations are B.P. 140/88, Pulse rate is 89, body temperature ...
