Introduction

Diabetes mellitus is a multisystem disease associated with abnormal insulin output, impaired use of insulin, or both. Diabetes mellitus is a serious health problem throughout the world through. This is the fifth leading cause of death in Australia. This is a major cause of cardiovascular disease, stroke, blindness, adults, and nontraumatic lower limb amputations. Background and Rationale:

This project is designed for people with type 2 diabetes in order to improve the devastating disease. It is aimed at the prevention of complications of the disease through the adoption of this community tertiary prevention programs.

Project Goal: Reduce morbidity and mortality associated with complications of type 2 diabetes in the elderly (65 and over) living with diabetes.

Goal 1: to identify people with type 2 diabetes and identify those with complications and those at risk of developing complications from diabetes.

Expected results:

• The number of type 2 diabetes aged 65 and over living in the area of study

• The number of type 2 diabetes with or without complications

• Common risk factors for complications
