The nursing guideline of SOAP (Eggland, & Heinemann, 1998) was used to construct the soap analysis. This guideline provides a systematic approach to evaluation of a patient and to take necessary action by the observations. It severs as a critical framework for nurses in gathering all the relevant information about the patient (Jarvis, 2012). The patient was analyzed and some psychological issues were observed in the patient. The eyes and ear observation was carried out and no medical issue was observed to be reported for consultation by physician.
Part A: Head, Face and Neck Assessment
Introduction: Caucasian 45 year old, with appearance of female gender. Weight and height seems proportionate and good posture. Facial and demeanor expression are suitable for setting in clinic. The female is dressed and groomed well indicating a well dressed personality which shows good self-care. The female seemed alert.
Subjective Information
Head
The head looks proportionate to the body and symmetrical to the features on the face. Integument is intact. The subject doesn't feel any pain or bulginess on the head.
Face
The face and the features on the face all look proportionate and placed at the right places on the face. The nose looks a little big but is not noticeable in the first sight. There is a by birth lip-cut on the upper lip. The forehead had continuous stress lines and the subject seemed distracted. The patient has shown some nervousness and sensitivity to the lip-cut and started telling about how she was unable to get it operated till now.
Neck
The muscle mass and the position of neck seem within tolerance. The mass seems to be distributed equally. The trchea is located at the mid position which is also the normal position of it. The patient expressed some pain as for the past few days have been very ...
