Medical Surgical Nursing Laboratory Workbook Labs 1-5

LABAROTARY 1)

1)

The medications most likely to raise blood glucose levels are corticosteroids. Physicians usually prescribe steroids to reduce inflammation and swelling. Steroids raise blood sugar similarly to the way stress hormones do, and some naturally occurring steroids act as hormones, such as the primary stress hormone cortisol (Reiss, 2002). Steroids cause the liver to manufacture extra glucose, and they block the hormone insulin's activity in removing glucose from the blood and into body cells.

2)

The relevant nursing considerations are as follows:

Ensure the patient can self-manage appropriately.

Advise patients not to stop this medication without speaking to the prescribing health care professional

Give smoking cessation advice if appropriate

Ensure patients know never to use this medication to treat an acute attack. They should have an appropriate short-acting reliever inhaler for this purpose

Give appropriate advice about maintaining the device

3)

The primary objective of a 'handover' is to provide accurate information about a patient's care, treatment and services, current condition and any recent or anticipated changes. Patient handover in the emergency department (ED) is mainly a two-way communication process between ambulance personnel and ED staff where information exchange is crucial for the transfer from pre-hospital to in-hospital care. The handover process with belonging reporting routines thus plays a key role in securing continuity, quality, and safety in patient health care. Standardization of handover will ensure effective, concise and complete communication in all clinical situations and facilitate care delivery (Moore, 1989).

4)

The three situations are as follows:

Admitting the patient

Treatment of the patient

Discharge of the patient

5)

The discharge staff will not be familiar with all aspects of your relative's situation. As caregiver, you are the "expert" in your loved one's history. Some of the care your loved one needs might be quite complicated. It is essential that you get any training you need in special care techniques, such as wound, feeding tube or catheter care, procedures for a ventilator, or transferring someone from bed to chair (Ramont, 2005).

6)

A medical practice's patient record serves many purposes but its primary function is to plan for patient care and provide for continuity in information about a patient's medical treatment. In my experience auditing medical records from the family medicine offices participating in the TransforMED National Demonstration Project (NDP), I have observed numerous accounts of medical records which were sub-optimal or had important components that were missing (Swearingen, 2007).

7)

Highly dozed and highly reactive medicines should not be given

The patient should be monitored on frequent basis

Ask the patient for the missing information (Gibson, 1999)

8)

Double-check all mathematical calculations for preparing and administering medications.

Review order, calculations, and verify dosage of highly toxic drugs with another nurse.

Check medication name, dosage, frequency, duration of therapy, and route of administration with the provider's order.

Compare medicine/labels three times to the order and/or medication administration record

Administer only those medications that you have prepared personally or received prepared from the pharmacy. The nurse is legally liable for all medications he or she administers; carefully examine all drugs and unit dose ...
