Introduction

A person centred approach in nursing facilities aim to focus on individual requirements of the patients, their desires and wants in order to make them central to nursing process and care. This means that the nursing approach puts the needs of an individual as they explain them above the needs that are identified by the healthcare professional as priorities. The person centred care indicates a string interest in the experience of the patient regarding his health, injury, illness or needs. It infers that the nurse carries out his/her work considering the definition of the patient about the situation along with medical diagnosis (Clouston, et al., 2003, pp. 136-42).

There are several frameworks which have been developed by the academics of nurse to facilitate the practices of nursing process in the implementation of person centred care. While there are slight differences in these frameworks they share some key factors:

Know the patient as an individual

To be responsive

To provide meaningful care

To respect the needs, preferences and values of an individual

To foster trust in care giving relationship

To emphasize on freedom of choices

To promote the emotional and physical comfort

To involve the family and friends of an individual appropriately.

In this paper discuss a case of a patient with diabetes is discussed together with literature review on person centred approach. Furthermore, the paper summarizes the discussion. In section A, I have conducted an interview with a patient. In this section, the paper discusses the experiences of the patient regarding the ageing process and the paper will also provide appropriate framework of person centred approach for this patient. In section B the paper discusses the current literature review on Person centred approach of the nursing care for aged care and its significance in nursing process and healthcare (Michie, et al., 2003, pp. 197-206).

Critical Reflection of the discussion with Patient

It important for person centred approach to communicate with patients and cooperate with them together with concentrating beyond a specific disease. It has been shown by some evidences that patient centred care improves the satisfaction level of the patient and it reduces the anxiety and increases the quality of life. This section provides critical reflection of the discussion with 65 years old Dutch Man George who lives with his wife.

It has been analysed that, George is a patient of diabetes and he suffers from problems of sleep pattern and vision. He requires having pharmacological treatments and counselling including specialists care, physical therapies and preventive strategies. The core perspective of the patient centred approach for George is an effort to interact with him in planning and development of goals for his health management. The aim of this approach is to enable him to disclose his fears and issues together with provision of support and counselling for the development of his own management tactics for his diabetes.
