Clinical Problem

Medication errors are commonly defined as any preventable events that may cause or lead to inappropriate medication use or patient harm while the medications are in the control of health care professionals or consumers. Medication errors caused by consumers are usually adherence and compliance errors and are not considered in this entry. Medication errors can occur anywhere in the medication use process, including the prescribing nursing staff.

The objective and reason of the study is to supply the explanation and description of the impact of the current nurse to patient ratio on the medication errors. This ratio also helps judge the influence on the performance and working in the nursing profession. Thus, the main objective of the study is know quality and the provision of medication to the patients while maintaining a certain nurse-patient ratio in the profession (Barker, 2002).

Patient safety is a central concern of current health-care delivery systems, and several recent studies initiated by the Institute of Medicine (IOM) have reported the high incidence of medical error and large gaps in health-care quality including medication errors, postsurgical complications and infections, inadequate cancer screening, inadequate care following a heart attack, and patient death. Of the approximately 44,000 -98,000 patient deaths reported each year because of medical errors, 7,000 were attributed to medication errors.

Even with agreement regarding the definition of a reportable error, accurate identification and reporting of medication errors are critical factors in the development and testing of medication error reduction interventions. Unfortunately, identification and reporting are often far less than complete. In a recent study comparing direct observation, chart review, and incident reports for detecting medication administration errors in 2,557 doses on which all three methods were used, 476 errors were reported during direct observation, 24 errors were noted in chart review, and 1 incident report was filed.

Rationale for Selecting the Research Question

To construct on the information groundwork that presents for living methodical reconsiders, a second stage of investigation will engage in categorizing them into three assemblies (representing distinct grades of power of evidence) to illustrate productive advances, undertaking advances, and advances that offer variable or no thriving checks lead to better conclusions for young children, youth and / or their families considering the emotional and mental health. Only investigations of dropping in the first two classes are encompassed (Weingart, 2000).
