Introduction

The focus of this assignment is on the healthcare assessment and care plan. Healthcare plan is available for anyone who wants to have a plan. It is an agreement between health professional and the patient in order to manage one's health in day-to -day life. It can be drafted in the form of patient notes or as a written document. Patients are encouraged to participate in drafting of care plan. This plan is an important instrument in assessing the needed care and how it would be provided (Baggott 2000, pp.14). The plan may include following items:

The ultimate goals of assessment and care plan such as recovering from addition

It mentions the support services which are required but the patient, and the person responsible for providing healthcare service and how these services will be provided

It includes contacts of related people such as physicians or other authorities, which are needed in times of emergencies

It describes medicines and schedule of taking medicines

It may include diet plan, exercise schedule or any other essential prescription (Baggott 2007, pp.3).

The next part of assignment covers the underlying polices of certain care plan for certain patient and disease. The last part of assignment will cover the issues related to relationships and governance issues (NHS.uk 2011, web).

Underlying Policy for Disabled Children

The disease, which is assessed under this assignment, is care of disabled children. The care of disabled children is governed by the Children Act 1989 (UK) (Books 2006, pp.13). If one cares of a disabled child, this care is governed by Children Act. The assessment of child in need is the duty of local authority, social service department under this act. Child in need also includes disabled or special-children.

The Children Act of 1989, which got effective in 1991, introduces complete alteration to previous legislation in Wales and England, with implications for the wellbeing of children. This Act establishes the freedom of families by comprehensively defining responsibilities of parents. It additionally provides support of community authorities especially for those families which require support for children (Means & Smith 1994, pp.24). The act legislated protection of children who are open to harm and who are already suffering from some harm. There is a requirement that government must submit an annual report illustrating the progress of the child act. The report must be submitted every 5 years. The report should also contain child protection statistics (Aldgate & Statham, 2001). The Act has following aims:

To converge public and private law into a single framework

To provide a vehicle to parent, that will allow them to challenge state intervention and achieve better child protection.

To promote better partnership among parents and statutory authorities.

To encourage application of voluntary-acts

To re-design the court framework in or to enhance family proceeding and protection management.
