Introduction

In the healthcare facility, it becomes very challenging for the family and the care takers of the patient to properly understand their need and make informed decisions so that appropriate actions could be taken in times of crisis situations. Risk Management strategies are imperative to be formed which are tailored according to the needs of patients and the suffering they are going through. For these to be effectively formed and implemented, outcome based practice is promoted in the arena of social and healthcare provision which focuses on the achievement of desired outcome; in contrast to needs-led assessment which focuses on the needs of patients.
