Safety in health care tends to define the measures taken health care management to avoid and control the risk of unintended negative consequences out of negligence. Likewise, quality health care aims to provide a standard concept of outcome in general. The requirements and standards of the two dimensions improve and refine with the passage of time in an organization / state along with the enhancement of knowledge and experience. The two subject matters can be segregated in the broad spectrum in the healthcare industry. Safety care carried in a healthcare organization cannot guarantee the quality care. The reason stands in the chance of error that threatens safe care and quality care both. The dimensions of these errors vary in scope like, diagnostic, treatment and even preventive. Therefore, in my understanding awareness is the basic element of quality care and safety care (AIHW, n. d).

Projected Quality Indicator:

Most of the errors that occur in healthcare industry are not documented officially because of the fear of unforeseen, lack of quality management and unawareness. Therefore, the most important step for a refined quality care and safety care is an element of education and automated surveillance. This step will bring in the invisible quality improvements in every field and department of health care industry by controlling errors.

Rationale of Mandatory reporting system:

Mandatory reporting system can be of great help in achieving the quality management system in healthcare industry. Evident is the fact that in health care industry each of the team members knows well his / her job responsibility and that of the other colleagues. Therefore, in case of any error, the team member can easily analyze the situation and highlight the concern responsible as the root of created issue. Here, the point of concern is that the error should not be taken as to begin the blame game, but simply should be followed as a measure of quality management to be followed for bringing in improvements in health care organization. In addition, this information will be equally beneficial for genuine feedback to the regulatory bodies on grounds of practical evidences to attain certain decisions (Cohen, 2000).

Key Element Involved:

The key elements required for successful introduction of the quality management is to define a simple reporting system, create awareness and coordination with all the departments of health care organization, with an option to obtain feedback on the reporting patterns as well. Reporting could be voluntary or mandatory as agreed by the Board of Directors. However, complete data tracking, and options for document review should be available for automated surveillance. Special consideration and emphasis should be led upon medical care teams engaged in patient progress. Hence, this step could provide authentic information on the persistent errors of healthcare industry and help in identification of preventive measures for specific circumstances.
