Explaining Outcome Based Practices

Beginning this section, it is only prudent that the basic meaning of the terminology, Outcome is discussed foremost. It means the array of possibilities or output that result from an activity, hence, associating outcome with results would not be wrong. Although, it is important to distinguish between outcome and output as these are highly interrelated terms. Outputs are the actions that are carried out by an individual to achieve a result or a service that is delivered by an individual. Thus, outputs are a means to achieve outcomes. While outcomes are the changes or benefits that are incurred from the output activities. Pertinent to our case, outcomes play a crucial role in the area and focus of community development. These outcomes help both the community leaders to use their knowledge skilfully and effectively in order to deliver effective change, which helps improve the efficacy of community life.

It wasn't till the early 1990's that all health care providers had discovered new and powerful tool that helped them in greatly diminishing the variation in the patient care practices, which is called the clinical path. However, it was soon that problems began to emerge from this particular approach. This led to these healthcare providers to adopt Outcome Based Practices that helped them to achieve their desired patient care objectives and goals. These practices are usually also referred to as outcomes management, which involves a collaboration or combination of continuous quality improvement, outcome measurement, processes and teamwork. These multi-functionary collaboration helps build pathway for further development of the healthcare system and operations in the early 90's.
