Dementia is a global mental impairment affecting all psychic abilities and gradually altering the emotion and the voluntary activity of the patient, his social behaviour. It is characterized by a progressive evolution of this deficit forever, dementia is usually due to organic brain damage, more or less diffuse nature abiotrophique, vascular, infectious, traumatic, toxic, or tumour. Dementia is one of the most feared and costly diseases in society today. It is defined as a clinical syndrome of acquired cognitive impairment that decreased ability determines intellectual enough to interfere social and functional operation of the individual and their quality of life. Many diseases can cause dementia, some of which may be reversible.
Mr. John who is suffering from dementia requires complex needs of care which motivated me to take a care study with diagnosis of dementia and who requires comprehensive nursing and multidisciplinary care for the treatment process. As a nursing student, I felt caring for Mr. John will be a good opportunity for me to demonstrate how my nursing skills were adapted to UK standards. Furthermore, in this care study a fictitious name has been used to protect the confidentiality of the patient and consent has been taken to respect his right.
Discussion
Dementia can cause language disorders, attention, concentration, perception and can affect memory. They are progressive and settled gradually, insidiously and lead to a complete loss of autonomy or keep the option to choose for oneself what one wants to do. Mr. John is 65 years old gentleman, he is unmarried, and he is a third child among five children of his parents. The mother and father of Mr. John were departed from this world during his young age. Mr. John is having a close relationship with one of his younger brother, who visits Mr. John frequently. At the age of 45, Mr. John diagnosed to have dementia problem. As an adaptation nurse, I have realised that to provide a quality care to Mr. John I have to have a thorough understanding about his illness. The dementia is defined as the disease in which impaired memory and other intellectual abilities with respect to the previous mental state of a person. The diagnosis of dementia must meet criteria to be established. It is said that dementia survivors will likely have a number of language disorders, attention, concentration, perception and memory.
In John's care, I have used the nursing care model of care 300. Nursing care 300 is comprised of assessment, development, working and evaluation of care provided to the patient. I realised that the assessment is an integral part of the treatment process. Therefore, I have gathered information about Mr. John's present emotional problems and physical health, and past medical history his from his brother.
