Description

Nursing assessment is a process in which information is collected about the physiological, psychological and sociological status of the patient. Assessment is the first process here in which the nurse carries out a detailed nursing assessment of the patient to identify the problems of the patient. A certain step has to be followed in order to deal with the wound effectively. The following paper identifies the process for dealing with the wound successfully and dressing it according to the prescribed measures according to the wound being assessed.

Feelings

Assessment is the first step in analyzing and managing a wound. A holistic approach to wound assessment and dressing is appropriate where proper patient care is given and the wound is carefully examined on the basis of which the best ways for treatment are discovered.

This process usually involves analyzing the wound and the patient. This is followed by identifying the pathology for dealing with the wound and after sometime the wound is reassessed to see if the treatment has worked properly and has healed the wound. If the treatment is working as planned, then the treatment continues, else changes are made and adjusted to heal the wound.

Evaluation

Wound healing treatment is planned according to the general health status of the patient. For this reason, it is necessary to carry out the comprehensive assessment of the patient in which patient's health is evaluated and then the treatment is planned and implemented carefully (Worley, 2004).

Analysis

The nurse is required to note down the following details when conducting patient assessment:

Full medical history of the patient and previous diseases, immune system, any disorders and allergies

Medications taken

Nutritional intake of the patient

Quality of life

Hereditary diseases and health issues
