We live in a post-modern society, where health and social welfare viewed in a single paradigm of “one size fit all,” which is inappropriate and badly chosen. Because, these huge grants and national schemes that won't help people which they intend to. In the past, health stresses on issues of health; whereas, social services have act in response to the increase in problems of social. However, it is apparent that there is a close association between poverty and illness.

This context transforms the philosophy of partnership towards mutual and collective collaboration, and even makes it necessary. The key reason behind this the clients' problems, entailing an input from several services, can be more essential in intending services in comparison to the centralizing and traditional distinctions between community nurses and social workers. However, under new relationships and philosophies, both social and health care works in collaboration to offer a flawless “one-stop shop” to fulfill needs of clients (Doran, 2001, pp. 390-393).
