WHSO ASSESSMENT REPORT FORM
Workplace Health and Safety Act 1995
1. Hazard identification, risk assessment and control
That the organization ensures that hazards are identified, risks assessed, control measures implemented then monitored and reviewed for effectiveness.
Employees of the bank are not consulted to take what constitute safety issues from their perspectives (systems are developed initially with the experiences of the senior management who were not senior once. This has stopped the improvement process in the system. Employees are the best people to identify these issues as they are directly involved with all the systems and procedures which can expose them to some risk).
There is no formal system in place which analyses past incidents and recommend corrective measure for their prevention in future. (Upon asking, bank management acknowledged that some incidents have taken place in the premises of the banks but failed to provide data on incidents related to health and safety of workers in last 1 year).
There is no formal accident response plan in Champbank. (Last time a hacker attacked the system and got all the credit card information of the customers. Bank got the system back in 1 day as they didn't have programming experts who could have get the system back immediately from the experts. This caused much damage to the bank as well as exposed all the customer information to the third party who can use it in quite a few dangerous ways to inflict harm to the customers).
Recommendations
- Implement procedures to properly manage exposure to risks (WHS Act 1995 s. 27A; WHS Risk Management Advisory Standard 2000; AS/NZS 4360: 2004 - Risk Management).
- The organisation should have an accident response plan meeting the requirements of the First Aid Advisory Standard 2004, s. 2.5 and emergency procedures complying with AS 3745:2002 - Emergency control organization and procedures for buildings, structures and workplaces.
- Workers must be provided with information regarding the accident response plan and emergency procedures (First Aid Advisory Standard 2004, s. 2.5; AS 3745:2002 - Emergency control organization and procedures for buildings, structures and workplaces).
- Records should be kept of work related injuries and illnesses (First Aid Advisory Standard 2004, s. 2.7)
2. Work environment
That a safe and healthy work environment is maintained.
Multi-tasking due to workload is causing health risks to the employees of the bank. For example, receptionist of the bank has she type and answer the telephone at the same time. She holds the telephone receiver to her year by twisting her neck awkwardly for a considerable period of time and she does it quite frequently. This can surely lead to the musculoskeletal injury to her neck.
There is no first-aid kit in the bank if someone suffers from an injury due to getting slip, trip from the wet floor or climbing stairs etc. floor of the banks are somewhat slippery too which made it more important for this ...
