Introduction

During life time; the societal, physical and psychological mechanism can be damaged due to some reasons, on which the individual need someone to support and look after him. Nursing care is a service to human that is provided for the purpose of healing biological, psychological, behavioral and social areas. It encompasses the entire aspects on health-illness scale that range from care during illness, health promotion, prevention of disease, provision of comfort and assistance in adjusting to changes of health matters. Nursing integrates the knowledge of medicine that combines with my passion of caring patients and respects their dignity; therefore the knowledge of pharmacokinetics has become important for caring old age patients. The aim of this assignment is to identify polypharmacy's role in nursing practice and how nurses can monitor and manage patients with polypharmacy. Moreover, I will also evaluate the outcomes of outpatients in elderly that would help in creating the better understanding of contemporary nursing practice.

This paper will also develop a clear understanding that would assist nurses to avoid inappropriate medications in elderly patients. Usually in nursing practice, we find that medications do not match with diagnosis. This is certainly a serious issue because generally nurses give wrong medications to patients that creates huge problem. This paper will cover the role of nursing, different methods of caring patients, and the responsibilities of contemporary adult nursing practices under the process of pharmacokinetics and pharmacodynamics. Moreover, it will also discuss the use of analgesics such as Opioids in pain management in the elderly care setting. At Health centers in US, nursing services are the responsibilities to understand the needs of patients and to offer the comprehensive care program for the well being of patients.

Pharmacology and Medicines Management

Pharmacology is a branch of biology and medicine that is related with the study of drug action that what physiological effects it have on tissue, cell and organ. It is very important to understand that the selection of medicines is only one step ahead for quality health care, but selection also needs to be complied by its proper usage. Every individual must receive proper medicine, in adequate quantity with proper follow up and information.

Under the Social Context

In the year of 1977, 'WHO' introduced the concept of essential medicines for public health care. These medicines are those that meet the priority health care needs of social system. They are particularly designed according to public health relevance, safety, efficacy and cost effectiveness. In current times, social health care system are greatly challenged in delivering better quality care without using extra ordinary resources.
