COPD Hospital at Home Care Research Paper



Abstract

The hospital at home care for the COPD patients to decrease mortality or re-admission rate in patients with exacerbation of chronic obstructive pulmonary disease to the hospitals require the nurse interventions plan on the basis of evidence base practices to improve patient health outcome and decline the readddmission rates. This also include the proper care in the hospitals before patient discharge and the discharge care plans for the patients to the prescriber nurses to keep therapeutic treatment and management carryon in the home as hospital care system. The interventions are required to make by professional and trained nurses to manage patient care in the emergency conditions to decrease the COPD severity patient re admittance to the hospital.

Introduction

In today's modern world, there is a fast growing requirement of educated and professional nurses to provide care to hospital at home patients to avoid readmissions due to exacerbations. Despite of extravagant medical and biological sciences advancement related to patient care in hospitals and at homes, there are still some concerns related to exacerbation of diseases that increases readmission to the hospitals. This disease exacerbation results due to various factors including the improper nurse care and prescription practices etc. This research here aims to study and evaluate the rationale behind the population elevated readmission rates and frailty increases with chronic diseases after hospitals at home care in geriatric patients.

Coronary obstructive pulmonary disease (COPD) is an obstruction in airflow with FEV1/FVC ratio less than 0.7, typically progressive in long term. The predisposing factors for COPD involve smoking, occupational exposures, however, frequent recurrent symptoms relapse results in exacerbations. The patient centered care is required in chronic COPD geriatric patients with hospitals at home care. This care is base on good interaction between patients and professionals (NICE, 2010, P. 6).

Evidence Base Practices and Practice Innovations

The introduction to the clinical nurse specialist (CNS) at the hospital has proved effective through several clinical trials evidences. The nursing evidence base practices include 5 stages, the practices intervention questions, the research review, its critical analysis, evidence application and reevaluation, to improve patient care at home hospitals to decrease the hospital readmission rate. These include discharge plan interventions and follow up care plans to attain broader success in readmissions decline through medicines management and adherence to medicines to improve patient outcomes through the health care setup (Hubbard. C, 2012, P. 1)
