Executive summary

The literature study which has been taken for critically evaluation and which can be implemented in my rehabilitation and palliative unit is Barriers to effective discharge planning: a qualitative study investigating the perspectives of frontline healthcare professionals. The primary objective of the study is to explore the current discharge planning process in the rehabilitation and palliative unit. The research will be very beneficial for the rehabilitation and palliative unit as it will help in analyzing and implementing an effective discharge planning process. The study will also be very useful in determining the importance and significance of communication between health care professionals and patients and how it should be improved for the future. The research paper can be generalized to other settings such as hospitals and other urgent care centers. If an appropriate and structure discharge planning process is applied and implemented it will be very beneficial for the rehabilitation and the palliative unit. An appropriate and systematic planning process will help in reducing the patient's length of stay at the hospital which will further reduce their costs. Introduction

As a nurse I am currently working in the rehabilitation and palliative care unit and providing high quality health care to patients all across the community. Rehabilitation and palliative units are responsible for addressing some aspects and dimensions of health and improving it with the help of many therapies and surgeries. The rehabilitation and palliative care helps patients to get relieved from various kinds of sufferings. In the rehabilitation and palliative units, doctors and nurses work to gather and utilize multi disciplinary approach and provide care to the patients. In today's era, more focus is not given to patient's quality of life. In Britain more than 55 percent of hospitals offer rehabilitation and palliative care so that healing and healthy environment can be provided and maintained.

The paper will critically analyze the research paper that will fit into my practice of area and how the research would be implemented and what kinds of potential benefits can be achieved.

Discussion

Currently I work in the rehabilitation and palliative care unit. I am responsible for managing patients who are more than 60 years old and have gone through surgical interventions for either minor injuries or through falls. Moreover I am also involved in the discharge planning process and work together with many qualified and professionals' doctors, nurses, social workers and nurses. We work so that patients can be rehabilitated and discharged to their homes. I also take care of patients who have less than six months to live (Crown, 2004, pp 123)
