Abstract

The care to the elderly population after the transition to home and nursing home from the hospital setting is crucial. Older patients following transitions, often readmit to the hospital due to inadequate care by nurses and family caregiver. The major problems observed include the medication error and sudden fall. To provide the effective transition care in nursing homes and residential place of patients, the presence of qualified, proficient and experienced nurse is important. The population of older people require unusual care based on the complications of their age and diseases. These demands have increased the challenge to nurses and thus require a nurse leader. A good nursing leadership in transition care setting can enhance the quality of patients care many folds. The role of nurse leader is to improve the awareness of nursing staff about geriatric care, and support them in providing better care. This paper discusses the flaws and their solutions to improve the health care in the nursing homes after moving the patient from hospital.

Introduction

With the increased age, the susceptibility of people to acute illness, life threatening and chronic diseases increases. The conditions of such older patients further exacerbate via their compromised immune system. Therefore, the elderly population needs the additional nursing care for better well being. The percentage of older individuals is increasing, which is increasing the need of nursing care and health care consequently (Salamone et al, 2013). Older people not only requires the care services in the premises of the hospital, but also entail the unusual care moving from a hospital setting to nursing homes or residential place. All these requirements of the elderly population have increased the challenges for the nurses providing the appropriate services of nursing and health care (Castledine, 2011). To fulfill the necessities of the patients, the services of competent and skilled nurse are crucial. This is more exigent and challenging task for nurses with respect to the increased demand of nursing services for the elder individuals (Doherty, Davies & Woodcock, 2008). The challenges for nurses in the current picture the qualified, competent and adequate staff of nurses to provide the required nursing services in hospitals, as well as at home. For this purpose, Finkleman (2012) says that nurses must recognize their vision, value of responsibilities and functions for all sorts of nursing services required by the elderly patients.
