Introduction

Nurses play a vital role in assessing the health of a patient, which includes mainly obtaining information that is more of mental, physical, emotional, social, and spiritual in nature. This information is considered very much important in determining the correct state of the patient's health. This is considered as the very first phase carried out in nursing where the basic motive of every nurse should be to identify the crucial needs and requirements of the patient. The main idea of this tactic is to determine the nursing related issues of a patient (Harkreader, H,; Hogan, M., A., 2003). Since, I was placed in a mental health assessment ward in the Hospital; therefore, I was responsible for maintaining a high quality of care for such sensitive and intense patients. The safety of patient is a great responsibility and nurses are actively engaged and involved in taking care of patients directly. Therefore, it can be said that nurse's involvement in health care system improves overall structure of the health care settings and also prove to be a beneficial factor with respect to patient's health (Society of Urologic Nurses and Associates. 2009).

The patient that is being assessed during the placement was a lady who has been suffering from dementia hip injury. The assessment report was aimed at examining the needs of the patient and quality of care from a nursing point of view before discharging the patient at the wish of her husband. The patient was observed to require elevate assistance while eating due to vascular dementia hip injury. The other essential needs of patient include specialized bed that is suppose to be operated by other helpers, crushed and soft comfortable foam for bed, and care taker available all the time as the patient was unable to move herself or speak loudly.

Discussion

Analysis of disease

The patient that I came across in placement was a female suffering from dementia hip injury. Dementia occurs is a result of brain non functionality. It mainly impacts one's ability to think, memorize, judge or behave (Luc, J., 2011). The patient observed during placement also had hip injury, which becomes very serious especially when patient is of older age. The extreme care becomes necessary in such cases as mortality rate may also increase. It badly affects one's ability to move and walk. However, research has shown positive improvements in people with dementia and hip fractures of moderate age as recovery becomes faster.

The pain from hip injury is considered acute enough amongst older adults and it is commonly found in cognitively impaired clients. Nurses play a key role in acute pain management, which involves paying full attention to patient. Since the patient was old enough so she also had a problem in communicating rightly and in case of insufficient pain management by nurses such patients' life may be considered at higher risk.
