This paper will underscore and discuss in detail the impact of the disease on Tony and his parents. Secondly, the paper will comprehensively elucidate the psychological and social issues that they both experienced during and after hospitalization. Lastly, the paper will focus on how the nursing competency, specifically domain 3 of the NCNZ impacted on nursing care to Tony and his parents.

Introduction and Clinical Conditions

The patient, Tony, 8 years old who arrived to emergency department with abdomen pain and severe constipation, he got history of Hirschsprung's disease, and a colostomy operation was performed on him during hospitalization. Hirschsprung's disease is a rare congenital disorder. The developement of the disease leads to prevention of movement of the by-products of digestion usually faecal material (Rodgers, 2012). Consequently, affected individuals show signs constipation, and in severe cases it may lead to complete obstruction. Tony's parents have another one year old son, who is being taken care by a family friend. Tony is admitted with this condition following intense pain in the bowel and constipation. The information presented in this paper was obtained through interviews conducted to Tony's parents as well as the clinical notes in the hospital's staff room. This information was discussed in consultation with my nurse partner.

Description of Disease

Nursing care and assessment are vital during the preoperative and postoperative stage in treatment of Hirschsprung's disease. Hirschsprung's disease is a rare congenital abnormality. The disease affects specific cells of the nervous system called parasympathetic ganglion cells located in the large intestines or colon (Rodgers, 2012). Under normal circumstances during the development of the baby in the uterus, the nerve cells grow along the intestines towards the intestines. In infants who have Hirschsprung's disease, the nerve cells stop growing very early while in the mother's womb (Haricharan & Georgeson, 2008, 266-275). These ganglion cells are absent in the rectum, and to some extent also in the colon (Raythorn & Ingebo, 1999, 164-166). The disease is a rare condition with an incidence rate of 1/5000 live births. The condition may bear a genetic predisposition. According to Rodgers ( 2012), the disease affects more male children than female. Children with trisomy 21 or Down's syndrome have a 40% increased risk factor of developing Hirschsprung's disease (Witvliet, et.al, 201, 39-43).

Clinical Condition and impact on Tony

Informed consent was obtained from Tony's parents in conducting open ended questions about the experience they had gone through during and after diagnosis of the condition. On their son's health, they were concerned about anastomotic leak and bowel obstruction, which is the breakdown on the anastomosis, which leads to the bodily fluids to leak (Haricharan & Georgeson, 2008, 266-275). The parents told me how the symptoms of these conditions had manifested themselves. This is because the patient suffering from this disease suffers from increasing pain, fever, toxicity, blood in stool and peritonitis (Craft, Gordon & Tiziani, 2010). The same was the case with Tony.
