In the health proportionately, the ethnic and racial disparities have strong effects on the minorities of America. However, in the reduction of the maternal and infant mortality there have been made marvelous and incredible achievements still these diminutions support some of the racial groups more than others.

During the 20th century, spectacular and vivid decline in the maternal and infant mortality was observed. In spite of this, one of the major confronts that are faced by the healthcare of the United States is the perseverance and diligence of the disparities in the maternal and infant health between the diverse ethnic and racial groups.

However, the actual reasons of these continuing racial disparities are still unsolved. The differences in the maternal risky behaviors, socioeconomic status, prenatal care, perinatal infection, and psychosocial stress do not sufficiently direct for these disparities (Anachebe, 2006).

The intermediate reason of the infant morbidity and mortality is the premature birth. It is transitional in the birth as precipitately in gestation outcomes from the immediate dysfunction in the uterine milieu. The prematurity is a continuum in which the entire population of birth before the gestation of thirty seven weeks experience some elevated risk and perils (Kramer, & Hogue, 2009).

At present, for the premature birth of infants there are no standardized and uniform practices for the follow up care and hospital discharge. Consequently, the babies that are born prematurely have to leave the healthcare after the birth without proper follow up care and discharge plans. On the contrary, the babies should be ensured to receive proper care before they are home so as to address their special needs of health.
