Reflection in Action - During

My own perception is that the most challenging and yet still rewarding for Nurse Practitioners (NP) is to open an independent practice office while filling the gap created as a result of diminishing or rather vanishing of the service by the nursing homes mainly the care physicians, as well as inadequate geriatric physicians.

Elders chose to remain at home and prefer to stay away from institutionalized care - nursing home, Singh, D. (2005). They are worried about their well being and the lack of quality at these facilities. According to Singh (2005), by 2030, 20% of the Americans will be at the age of 65 or older. Therefore, a solution is needed to serve the elder care have never been desired before.

According to Singh (2005), Certified Nursing Assistant (CNAs) is among the jobs with the highest turnover rates. This might be due to a very demanding nature, long hours with lower pay rates, Singh (2005). Yet, nursing homes are losing larger portion of private pay market to Continuing Care Retirement facilities, Singh (2005). Therefore, it is important for NPs to provide service for elders.

It has also been observed that enough medical students are not choosing primary care physician (PCP) or Geriatrics as a career. According to Butler, R (2000), the current need is estimated at 2,400 academic geriatricians yet fewer than 600 physicians in the United States today have the requisite medical, academic and scientific training to teach geriatrics to upcoming generation of health care professionals.

Business location has to be the first step for opening the practice and key factors to evaluate include gender distribution, age distribution, economic distributions, cultural and language distributions as well as the business structure that has to be defined first.
