Introduction

Health Promotion, Health Behavior practice, effective public health programs and programs for the management of chronic diseases help people and the population to improve their health, maintain a healthy lifestyle, and minimize the risk of disease onset and also the management of existing diseases. Not only on the individual level, these measures are proven to be effective in improving the community health and self-sufficiency and wellbeing of the individual, family, organizations, community and society. The result is a healthy nation living a disease free healthy life. The effective implementation of chronic disease management programs needs the change of behaviors not only at the individual levels but also at every level of society (U.S. Department of Health and Human Services, 2005)

The implementation of all the health promotion and management does not ensure guaranteed success. The foundation must be based upon clear objectives and targets, understandings of the health behaviors and factors affecting it. Health care practitioners play a very important role in this regard as they use strategic planning models for the design, implementation and management of chronic disease and health programs. Health behavior theory can critically play an effective role in the program planning process (U.S. Department of Health and Human Services, 2005). Theory and Effective Programs

Theory is a tool used by the planners to move ahead, plan, evaluate and implement health behaviors and health promotion interventions based upon the health coverage of the population. It gives them a chance to step back and first analyze the situation on the mass scale. It is likewise a stepwise evaluation when a planner plans a program considering a health intervention and creates innovative ideas and ways to address the specific conditions. The standards of evaluation follow the authentic behavior theories which are skillfully applied to design the solution and solve problems.

The use of theory as the foundation in the program planning and management consistently put an emphasis on the interventions in public health, medicine, behavioral medicine and chronic disease management. The purpose of the theory is to provide a road map on how to study the problems, develop applicable interventions, predict the success rate and draft the best possible solutions. Theory also explains the changing aspects of health behaviors, including the ways involved in changing the, the influencing factors, the forces which affect the health behavior as well the social, physical and environmental factors. Theories also help the planners to identify the target population, the most prevalent disease, the methods of changes and the results (Weiner, 2009).

When a practitioner begins the planning process of developing an intervention to identify a health problem, or promote a health concern, or intervene a change in health behavior.
