There are various aspects which are provided in the paper pertaining to the health and medical assessment including physical health assessment of adult, multi disciplinary team and inter professional relationship within management and care of individual with long term conditions, basic or essential needs of adults and older people, basic care requirement to children and young people to meet essential needs, essential mental health needs when they exist alongside other health needs, compare and contrast of challenges to personal safety within range of care setting.

Discussion

Promoting health is an essential element for economic and social development and helps prevent health inequalities. Awareness about the difficulties of access to health for the population excluded socially vulnerable is the first step. Health status is related to biological and social characteristics of the individual, economic, and political organization, social structure, and cultural background, as well as with macro demographic and ecological processes that must be taken into account for noticing trends long-term health and disease process (Adler, 1994).

The various settings related to health and social care policies might include personal and organizational health. Personal health also depends partially on the social structure of a person's life. A number of other factors are known to influence the health status of individuals In addition to health care interventions and a person's surroundings. These factors include the lifestyle, educational background, and economic and social conditions. The health is affected by many conditions such as poverty and provisions of relevant drugs in area. This is followed by the appropriate usage of medicine prescribed by the medical professionals. These health and social care policies prevent the misconduct of procedures and any deviation from the concerns related to health. However, the problem and challenge prevail in different settings.
