Discuss different models of health & health facilitation

The role of health facilitation has developed with time. The family carers and practitioners wish to improve the health of people facing learning disabilities. The relatives and patients of learning disabilities require support and navigation for their cure at its best and suitable healthcare. The health facilitation helps in the assessment of mainstream services to help people and also to help parts in NHS for the development of necessary skills within the mainstream services. The role of health facilitation involves service development work and informing planning and commissioning along with the work for learning disabilities from people to people. The construction of learning disability, like any identity, depends on the complex interaction of people, places and activities and as such, it provides meaning to that individual's action, which in turn creates their labelled existence (HM Government 2009, 5).

The socio-economic gradient includes obesity, smoking and nutrition. The socio-economic factors include income, employment status, socio-economic group and educational attainment.

In the first place, national initiatives have concentrated on improving services and have clearly recognized the problem of access for people with learning disabilities.
