My name is Angela Hatcher and I am a graduate student at Central Michigan University. For my final project, I am examining Veterans Affairs Services Utilization by Veterans who participated in Operation Iraqi Freedom and Operation Enduring Freedom. Because you are a Veteran who returned from deployment within 3-6 months, I am inviting you to participate in this research study by completing the attached survey.
The following questionnaire will require approximately 3-5 minutes completing. There is no compensation for responding nor is there any known risk. In order to ensure that all information will remain confidential, please do not include . Copies of the project will be provided to my Central Michigan University instructor and to Kim Gribben, Assistant Director, and Masters of Science in Administration Program. If you choose to participate in this project, please answer all questions as honestly as possible. The completed questionnaires must be returned promptly (this is an online survey). Participation is strictly voluntary and you may refuse to participate at any time.
