Discussion

Being a coder is no easy task, to analyze and evaluate the patent charts and to allocate codes regarding medical diagnosis and specialty require constant drain of energy. On asking him about how it all works, he explained that coders work on the scanned documents and enter the details in a software that is designed for practice management. The details includes all the demographic and minor details such as name of the patient, his address ( city, zip code, state etc), contact information ( workplace, home, cellular phone, fax, emails etc), Social security number, job status, marital status, details of insurance and guarantor, charge entry etc. Other details such as date of service, place of service, referring physician, type of service, ordering physician, referral or authorization details, Co-pay details, code details (C.P.T Codes, Modifiers, I.C.D Codes) and locations are also added.

After the entry of demographic details, status details are also integrated into the management software. These are the details on the scan-control list, pending list and charge brief reports being sent to the patient. A query or issue log on any pending issue is also forwarded in order to address the claims (if any).
