The healthcare systems have an objective to uphold and perk up patient care with respect to its functional status, sense of well being and the disease. This aim can achieve through a strong inter disciplinary communication link between the healthcare providers i.e. physicians, nurses, psychologists and the patient and the interdisciplinary collaboration between the nurse and the patient. The health care facilities deliver mostly through the collaborative teams. This involve the understanding of healthcare providers function, accountabilities and proficiencies in relation to other fields to control the indistinctness and improbability and working in cooperation with each other to evaluate strategies and supply care.

The differences between intra and interdisciplinary teams
Interdisciplinary Team Model
The interdisciplinary health care team (IHCT) is a group of professionals that work in collaborative environment including physicians, nurse, pharmacist, and psychologist etc. to support and resolve patient issues that are complicated by any single discipline. Intradisciplinary Team Model
The Intradisciplinary teams include only one professional field involve in direct interaction with the patient i.e. nurses. The registered nurse communicates with patient and facilitates them through the primary health provision to patients individually or in communities. The interdisciplinary nurse must have progressive knowledge and decision making ability in the patient's health diagnosis, analysis, the management of healthcare, the interventions related to therapeutics which involve pharmacologic , counseling and complementary interventions, and community planning and development to improve patient's health.
