Implementation of EHR for Hospital Italiano de Buenos Aires HIBA
I. Introduction
The main purpose of this paper is to make an analysis on the implementation of electronic health record (HER) for Hospital Italiano de Buenos Aires (HIBA). HIBA was facing problems in the substantial flow of information in the homecare service. Therefore, the management of the hospital decided to make integration of all levels of care in the hospital information system.
II. Planning
Documentation is an issue of importance to nursing for many reasons. Quality patient care depends upon complete and accurate information among care givers. Documentation is also an essential component of the accreditation standards and legal requirements imposed on health care organizations. In addition, future costing of nursing care will be dependent upon correct documentation and the ability to link clinical information with financial and administrative data (Aguilera, 2010). Therefore, there is a great need of planning for the substantial flow of information on homecare services. Charting procedures must comply with standards of professional and accrediting bodies for the continued accreditation of nursing services. Charting errors can lead to liability, and potential lawsuits for individual nurses are well as for the hospital. In the U.S., hospital payment based on patients' medical diagnoses has made documentation for supporting primary/secondary Diagnostic Related Group (DRG) categories a critical issue for all healthcare providers.
1. Clinical and administration application
Electronic personal health record can be defined as the information of the patients that record on electronically rather than on paper. The access to the electronic record must be protected by access levels and password control. Clinical systems are required for the clinical records. All the patients' confidential information stores in the NHS separate drive and it have restricted access for the purpose of securing the privacy of the information (Pope, 1995). The clinical health care can be reasonable care, preference sensitive care, and supply sensitive care. In effective health care, the benefits overcome the risks. In preference sensitive care, there must be more than one treatment of the disease like patient can go for surgery or medications. The supply sensitive care comprises of clinical activities. These clinical activities are following: visit the doctors, diagnosis tests, and admit in the hospital. At the same time, care must also be delivered in a context of cost constraints, increasing patient expectations and a greater focus on accountability (National Patient Safety Agency, 2007). Same measure of the indicators can serve multiple purposes such as a healthcare professional to see how they are doing. It allows patients to think about their chances of getting better treatment and choosing the best hospital and doctor. And finally the society on how much it is going to cost to improve the quality of care.
