In the U.S. health system is divided between the private and public. However, both health systems, both public and private, are managed predominantly based on the principles of free enterprise characteristic of the American system. In theory, this free enterprise system should provide efficient, affordable health services and a competitive environment that would allow a successful private health system based on the prepaid system or Pre-Paid Health (Managed Health Care).

However, in reality, the American people increasingly realize that the prepaid health care system is not providing the public access to quality health system. First of all, health is a factor that differentiates it from other systems such as education, defense, etc.. where the free market can determine to some extent the quality of services rendered. However, in the health system, the human organism is a consumer, participate and object of consumption of health, so that at any time one of these three variables can affect the other two.

For example, a human being is a consumer of health services, but the very fact of illness that can incapacitate a human being to work and generate enough resources to be the subject of or participant in health. On the other hand, when a person suffers from diseases that consume hospital resources, the person in question may have been a participant in their own condition because of bad habits such as smoking, drug use, etc. At each time, consumer issues, participant and subject themselves become clogged, and though some may affect others, the health system is rarely seen in a holistic manner.

Finally, in the United States free enterprise system with which the health system is managed has a number of fundamental contradictions that conflict with the idea of ??a health system fair and efficient.

1. Health determines the welfare of a human being at the most basic and fundamental, for this reason, according to WHO, "health has the characteristics of an inalienable right." Thus, an inalienable right cannot be based on laws of supply and demand, where resources are rationed according to the ability to supply and demand of each side of the market.

2. The free enterprise system where you "get what you pay can not apply to the health system. The ability to pay an individual cannot be the determining factor in the quality of health services you receive, since health is intrinsically linked to factors that money can not control, such as biology.

3. Although according to WHO research the most effective system of health financing is the system of prepayment, in the United States, almost 25% of health care costs are lost in administration costs and paperwork.
