NHS is divided into two sections; it includes secondary and primary care. Primary is a major concern among many people as it is provided to people by variety of independent contractors, these contractors include, Dentists, General practitioner, pharmacists and optometrists. The structure of NHS consists of Secondary Care, primary care trusts, acute trusts, care trusts, mental health trusts, ambulance trusts and Nice.
Secondary care is considered as acute healthcare service or an emergency care. The context of elective care means specialist medical care, which includes surgery, it is usually followed by the general practitioner, GP are community health professional.
Primary Care Trusts are responsible for providing primary level of care, they play a major role in order to commission a secondary care, and it also includes the services provided to community. The primary care trust controls around eighty percent of NHS budget. (Department of Health 2003 Pp- 223-267) Acute Trust are responsible for looking after around sixteen hundred NHS hospitals, currently there are around one hundred and seventy acuter trust, Ambulance trust provide the services of NHS Ambulance services, there are around eleven ambulance trust in England providing ambulances services. Care Trust of NHS provides care in both social and health fields, these care trusts are very less in amount. Mental health trust of NHS provide the mental health care in England, these are usually seen by the local practitioner. Finally NHS structure includes National Institute for Health and Clinical Excellence abbreviated as (NICE), this agency is controlled by the NHS.
Shake up Changes
There have been many shake up changes took place in NHS, government is carrying out major steps to the NHS in England. Most Shake Up includes the responsibility provided to general practitioner for spending budgets, with the help of this change hospitals gets free from central control. They have design an independent board which will look after services provided to patients, this plan is considered as one of the radical plan in the history of NHS. It is shake up change because with the help of this change around eighty percent of budget is left with the local managers controlling primary care trusts. Currently there are one hundred and fifty one PCT's in England, and these PCT's are effectively in charge for commissioning domestic services, it includes hospitals, general practitioner, community clinics and mental health units. (HENAGHAN, M. 2012 Pp- 143-190) With the help of this scheme much of the responsibility will be transferred to General practitioner working across the country in consortium. The changes affected Primary care Trusts, PCTs were affected due to the groups of General practioners, as from now onwards GP are responsible for controlling and commissioning most of health services including many other public health roles, under the social and health care bill.
