Introduction

In its simplest form, an electronic medical record is the medical document stored in the machine-readable format. Data are entered into record via many different sources, including computerized entry and various document imaging systems. The ideal electronic medical record should reflect same information that is typically found in classic paper record, with variance according to provider as well as medical.
