Medications play a pivotal role in healthcare but can also be a significant critical cause of medical error and adverse patient outcomes. Nurses by the nature of their roles in medication administration can be the last line of defense in eliminating or reducing adverse medication events. The administration of medication is a common and almost routine activity in a nurse's daily work, yet it is fraught with complexity and risk for both the patient and nurse. As a student nurse working in partnership with a registered nurse, it is observed a variety of practices in medication administration that have varied from what I have taught in class. On reflecting these practices and questioning nurses why such practice has adopted has illustrated both the flawed processes and environment that nurses provide in care (American Academy of Pediatrics, 2001). Nurses will need to develop an understanding of how the environment they work in and within the systems.

Those systems that are currently in place can impact on their ability to provide care in a safe manner. Nursing must play a crucial role in reducing the number of adverse medication events. As pivotal health care providers nurses will need to be examined their roles in leadership, patients' advocacy, technology and their role within the healthcare team to reduce the levels of harm currently occurring in our modern health systems. If nurses are to maintain their key role as patient advocates, they will need to step up and become more actively involved in understanding the causes of medication errors and be engaged in developing a culture of patient safety.

Thesis Statement

Nurses will need to develop an understanding of how the environment they work in and within the systems. That are currently in place can impact on their ability to provide care in a safe manner.

Definitions of Responsibility in Nursing

1.The image and role of nurses has undergone several transformations, it is like other professions, is based on a social concept and as a social activity takes place in a constantly changing historical context, and conflict. Their development has gone through a practice of care based on empirical knowledge, whose values determined by certain dogmatic ideologies, influenced by traditional moral, and models based on self-denial and sacrifice (Alldred &Standage, 2008).

2.Nursing live in a society in which values are reported, of a culture and the degree of economic development attained, but above all, the shape of the society. Hence the importance of training of the nurse (or) from the theoretical and practical point of view based on principles, norms and values.

3.Provide comprehensive nursing care, insurance, timely, humane and fairness to the individual, family and community groups, respecting their culture, religion, race, myths, age, sex, political beliefs, customs, beliefs and values.

4.Protecting the right to genetic integrity, physical, mental and spiritual person throughout the process of nursing care.
