Assessment of the Situation

According to the case, since, Mr. Trosack has been discharged from the hospital after hip replacement surgery. In this paper, we will discuss the care plan and assistance which can be provided to the patient after surgery. The health care issues must address when working with an interdisciplinary team is that there is a lag in communication between patient and doctors since the patient needs assistance and guidance of the doctors.

The pain management of a patient is important to understand as it helps patient to recover early. Since, the patient is under observation, so the medication must be used at the time. The pain should be prevented, and medications should also be check, and provided after complete assistance so that the patient does not get prone to medication (Pandit, 2008).

There are also cases of dislocation which is common in hip replacement surgery. As the replacement, results in the femoral head taken out of the socket and hip implants, placed to put the hip back into position. The surgeons should make sure that the patient checked so that the problem of dislocation does not arise. These issues are important when planning for management of Mr Trosack as an elderly patient who is discharged from the hospital and if these issues are ignored they can lead to problems for the patient.

The role of nurse in patient Trosack case after discharged from hospital should be provided treatment. Depending on the type and scale of the operation, as well as speed, the healing process, nurse should use crutches or stilts in a period of several days to several weeks. At the time of surgery, the nurse should ensure that the various maneuvers required care do not compromise the success of the intervention. The team must help the patient to the toilet and nursing care, explain to the patient how to raise, make the prevention of pressure sores and avoid complications of recumbent. Before turning the patient on the opposite side of the operation, a pillow should be placed between the legs. She should check that the operated limb is externally rotated, if not replace it gently and provide a standard radio control. They must remove the stitches within 2 weeks after surgery (St Clair, 2006). In the end, she should inform the physiotherapist to start rehabilitation as soon as possible.
