Introduction

Clinical judgement

The clinical judgement is a process by which the nurses recognize the actual presence of the problems, the issues and other factors related to the patients. In this process, the nurses attain the essential information of the patient and based on that information, they respond to the patients' issues and problems in strategic ways (Daniels, R., 2003, pp. 58). The clinical judgement is considered as an important ability of the nurses and this ability differentiates professional nurses from those who play their pure technical role (Gurbutt, R., 2006, pp. 30).

Introduction to the case

The selected case is related to the mental health scenario in which the Trevor, who is 47 years and a patient of the schizophrenia, was brought to the hospital for the fifth time. The patient is highly involved in the mistrustful and the aggressive behaviour and many times, the patient is irritating and hostile towards other patients as well as towards the nurses attending him. The case study mentions that there are four nurses, involving me as the nurse in charge, registered nurse, assistance nurse and two student nurses. From these four nurses, one student nurse has been given the duty to closely monitor and observe another patient in the ward. The ward is currently having sixteen numbers of clients.

The Carnevali's diagnostic reasoning process has been developed for the assistance of the complex nursing activities. This diagnostic process involves the amalgamation of the critical thinking, decision making, data collecting and the data interpreting processes that is used by the nurses in order to find out the current clinical situation. The Carnevali's diagnostic reasoning process offers the framework and the relevant clinical treatment decisions. The clinical treatment decisions are made in collaboration with the responsible nurses who make use of their experiences in drawing the conclusions. The Carnevali's diagnostic reasoning process involves seven stages with which the nurses come to the final clinical decisions. The case of the Trevor will be analysed using the Carnevali's diagnostic reasoning process.

According to Carnevali's diagnostic reasoning process, following are the seven essential stages.

Stage 1- Exposure of the pre encounters information

In order to provide the treatment as soon as possible when the client arrives to the hospital, there must be pre encounter information regarding the disease by which the client into question is suffering. As the client, Trevor, is the patient of the schizophrenia mental disease, therefore, the immediate nurses will have to collect the information regarding the occurrence and the situations of the schizophrenia mental disease keeping in view the medical history of the client.
