The document will discuss adult nursing care in regards with patients who constantly require the monitoring of blood pressure. Discussion

The blood pressure of an individual is the measure of how powerfully the blood is pressing against the arterial walls as it flows around the body and is pumped by the heart. The blood pressure of an individual is comprised of two measurements, the first one is taken while your heart is beating and performing the function of pumping blood where as the other one is taken while the heart is getting filled up with the blood. The first one is known as systolic blood pressure while the second one is known as diastolic blood pressure.

High blood Pressure is one of the most common abnormality affecting at-least one fourth of the total population in U. K. Often it goes un noticed as its symptoms are either very weak ones to be identified or are very similar to other illnesses. The risk of hypertension increases with the growing age of an individual. Hypertension occurs when an individual's blood pressure is always elevated above the normal measurement. As it is more frequent in adults the importance of nursing interventions regarding controlling and monitoring blood pressure in while caring for aged patients is highlighted.

A public health care interventions carries out persistent and wide-ranging practices that are rehabilitative, curative and preventive in nature. The public health care plan was carried out in obesity prevailing community, suffering from high blood pressure.

Reasons behind Intervention

The core responsibility of adult nurses is to care for the adult patients who are suffering from long term and acute illness. The nurses are responsible for presenting the patient with support recovery from the disease or sickness through the use of effective care delivery methods, executing assessments and procedures and by focusing on the needs of the patient rather than the illness or condition. Nurse practitioners typically do the job within the framework of multi-disciplinary teams but remain the main point of contact for the affected individual.

The morbidity and mortality rate in this community was viewed to be significantly higher than other population. This was because of high blood pressure and its leading consequences (such as brain hemorrhage or strokes). The plan was executed after profound research to ensure that at-risk population is informed about the serious consequences of hyper tension and is made to learn preventive measures.
