Introduction
This study is based on the nursing practices of registered nurses for any kind of patient in hospital. In this case study analysis we try to explore the concept of nursing practices in a holistic context. The main focus of the research is on the case study of a registered nurse, who has started a new job at the hospital. There she reviews a history of Dementia patient jack and its relation with the attitude of nurses and caring behavior at hospital. She realized that she needs to do the comprehensive assessment and develop a care plan for Jack.
The research also analyzes many aspects of nursing practices for a Dementia patient and tries to gauge its effect on patient history and care. Finally the research describes various factors which are responsible for causing the lack of care attitude and tries to describe the overall effect of Nursing Evolution on Patient care.
According to the research, conducted in United States, there are around 5.1 million people suffering from the case of dementia and approximately seventy percent of them are not obtaining medical treatments from a medical facility, they remain and have therapies at their very own homes where they're obtaining care and attention from their individual families as well as their friends.
Dementia
The defect of dementia is defined as the decline in the reasoning, mental ability (cognition) and memory of the patients. It decreases the ability of the patient to carry out the everyday activity like house hold works, driving, personal care, dressing or feeding.

People suffering from Dementia are kept under nursing observation
people with dementia also experience declines in any or all areas of intellectual functioning, for example, use of language and numbers; awareness of what is going on around him or her; judgment; and the ability to reason, solve problems, and think abstractly.
Case Analysis
I started off with my new job as team leader under the supervision of health care assistant, in a 60 bed hospital. There are 12 patients who come under my immediate supervision.
