The objective of this paper is to examine the health assessment of an 83 old elderly patient, including the choice the assessment tools and assessing case presentations and outcomes. The 83 year old patient is male who is dealing with dialysis, has only the emergency Medicaid and needs assistance with most ADL which could heal its medical problem properly. Newman system model (2011) is used for the assessment and decision as well as presentation of the case management as the model greatly supports the presentation of elderly cases in nursing and based on holistic model supporting the assessment through interdisciplinary team. The paper starts with explaining the history and assessment of the patient following with assessment process of the patient based on the Newman's System Model. The model will be discussed in case management including the identification of lines of defense, lines of resistance, client stressors, and variable. Further, the paper includes the need for a case manager and other healthcare professionals in multi-disciplinary team that is required in the healthcare settings. Moreover, the paper discusses the plans which could help understand the continuity of care and follow-up schedules as well as type of monitoring for future evaluations. I will discuss the case of 83 old elderly patients on dialysis and used the first letter of his name in order to protect the confidentiality of the patients as discussed in AMA's Code of Medical Ethics.

Introduction
Today patients are in increasingly great demand of several ADLs especially the geriatric and elderly patients who require the assistance in their clothing, meal, cleaning, toileting, medications, shopping, transportation and many other things which they usually unable to do themselves at this stage of their life. Many research studies have observed that needs of these ADLs are required when patient is experiencing the serious medical problems such as getting paralyzed, kidney loss, dialysis, and the like. They found difficulties in managing their daily tasks and if they found themselves unable doing so, they caught in serious stress related issues such as building the history of hypertension and the like. The case of Mr. A is similar to this and this will be analyzed through Neuman system model which help understand the case history and assessment as well as stressors, flexible lines of stress, normal lines of stress and lines of resistance.
History and Assessment Mr. A is 83-year-old African American male patient. Mr. A was admitted to the hospital after having kidney and is currently on the dialysis after the three days per week. He is Roman Catholic living with wife aged 70 years old and living in one bedroom apartment.
