Case History of the Patient

The patient is 65 years old. The patient's wife reports memory loss of six weeks duration, in the patient. The condition of the patient is worsening. The patient also feels convulsions and tremors of arms and grimacing of face. He seems to be well most of the times. There is no relevant family history of the patient.
