Procedures

What to do

a) Encourage older people and family members to participate in the planning process.

Tell them the agenda and all the actions to be performed in scheduled session of the planning meeting. Encourage them by providing full information about the meeting.

b) Conduct planning meetings.

Meeting room should be arranged with all necessities. Strictness about punctuality will lead to efficient planning meeting. Agenda should be cleared, and all the exercises should be made according to that agenda.

c) Develop and document individual plans.

To develop an individual plan, it is necessary to know about each and every problem of patient.

d) Assess risk.

Risks should be discussed and assessed to reduce the patient's stress level.

e) Protecting the confidentiality of information.

Information should be kept confidential by making a file or keeping that information in a locker so that no one from outside can access to that information.

f) Assign activities to workers.

A meeting should be arranged to assign all the projects to the workers.

g) Train staff.

3 days or 1 week training should be designed to exercise the expertise of team members.

h) Allocate resources

Resources should be allocated by getting information as per requirement.

Part B

1) a) Information about her performance is essential for the process. Either Delia is misbehaving with other family members, or she wants to be alone. Either the death of his husband led her to this disease, or ignorance from her children led her to this situation. Information about the daily activities of Delia will also be needed to assess the mental ability of the patient. Information is needed about any previous illness or disease, targeting mental health or any disease.
