Learning Outcome 1

Modes of inter-professional collaboration in the delivery of health services are important for improving health care focused on the patients. The World Health defines collaborative practice in health care when many health workers with different professional backgrounds provide comprehensive services by working with patients, their families, caregivers and communities to provide care of the highest possible quality in different areas of intervention (Moon, 2008).

Research believes that inter-professional collaboration modes for the provision of health services are important for improving health care centred of the patients in United Kingdom. The system's ability to respond can be enhanced through effective collaboration between health professionals, policy makers, educators and professional associations.

Study believes that the following principles will facilitate collaboration between professions and professionals.

Learning Outcome 2

To support and sustain inter-professional collaboration, the following structural elements must be present;

Planning, recruitment, work environment and inter-professional education to support human resources.

A long-term funding to support the infrastructure requirements and information technology for inter-professional collaboration.

A framework of liability insurance for inter-professional teams including insurance responsibility for the health professionals that are independent of the liability insurance of the employer.

A regulatory framework that allows every professional to use their knowledge, skills and its experience to work in any field of practice, recognizing the decision-making processes and roles in the context of inter-professional collaboration.

Standards that ensure both interoperability of electronic health records and access to them by the appropriate health professionals.

Governance structures and management systems that foster inter-professional collaboration and ensure funded health care.

Planning frameworks, assessment and evaluation tools to measure performance practices with interdisciplinary collaboration, which are supported by ongoing research and monitoring.

Studies show that inter-professional patient-centred collaborative practice can have positive effect on current health issues such as latency. It helps in promoting workplace health planning of human resources for health, patient safety, access to primary health care in rural and remote area, chronic disease management as well as health and well-being of the population. Successful collaborative can improve the patient journey through the health care system with good results for the patient as well as care providers and the overall system (Donnelly, 2008).

Learning Outcome 3

Health care focused towards patients

Health care focused on the patient; means that patient-centered health requires inter-professional collaboration with the clients. Nurses and other health professionals can help facilitate health care since they work across individual, organizational, or anywhere in the system of health care (Moon, 2008). Professionals of health combine their work to optimize the health and well-being of the client is consulted for decision. Each client is actively involved in prevention, promotion and management of their health.

Decision-making based on evidence for quality care
