Introduction

A health program is a set of actions taken by a government in order to improve the conditions of health of the population. Thus, the authorities promote prevention campaigns and ensure democratic access to mass care centers. Typically, a health program consists of several parts. In principle, there is an introduction to the history and mission to fulfill the program. After performing a diagnosis of the current situation, which may include assessment synthesis similar schemes have been developed previously (Coddington & Moore, 2000).

The Patient Centered Program

The patient- centered care is replacing the current physician centered system with one which involves around the patients. The effective care is actually defined as the in consultation with the patients either than through the physician dependents standards and tools. How was the program restructured or reengineered to adapt to internal and external factors impacting it?

According to Marci Nielsen, The patient -centered program is being restructured as, We propose a framework for understanding the process of initiating patient-centered innovations-particularly innovations addressing patient-centered goals of improved access, continuity, communication and coordination, cultural competency, and family- and person-focused care over time.
