In this study, we try to explore the concept of “Community assessment” in a holistic context. The main focus of the research is on “Community assessment” and its relation with “health care.” The research also analyzes many aspects of “Community assessment” and tries to gauge its effect on “health care.” Finally, the research describes various factors, which are responsible for “Community assessment,” and tries to describe the overall effect of “Community assessment” on “health care.”

Community Assessment
Introduction
Assessing the strengths and those that should improve in a given community is an important first step planning a service project effectively. By devoting time to in- formed on the problems of the community, the club may des- cover new opportunities for service initiatives undertaken and avoid unnecessary issues to be addressed.
Assessing the community is a very important and valuable strategy that one can use to get a clear and transparent picture of one's community. This thing will assist a person in categorizing the problems of the community and highlight them in a proper way. This thing will also function as the initial point for the development of communal vision and strategies that can help in encouraging change in a community.
The tool of community assessment helps a person to collect information from a number of different souls and groups within the community; i.e. different types of information will depend on the focus of partnership's and resources that a person can use to collect information. In this report, I will discuss my findings in the process of community assessment of Indianapolis community.
Indianapolis is a community that is located in the essence of the state of Indiana. The total area of Indianapolis is 368.1 sq miles i.e. 953.5 sq km. The per capita income of the city is $21,640. According to the US census that was conducted in the year 2004, the total population of Indianapolis is 794,160 (Boomhower and Dunn 1997).
Population economic status assessment
According of the census of 2010, the total population of Indianapolis was 829,718. The density of population in the county is 2,230 persons per square mile.
According to the census, there were 58.6% Non-Hispanic whites, 27.2% of the population were non-Hispanic and African American, 0.2% of the total population consisted of American Indians, who were non-Hispanics and native Alaskan, 2.1% population were Asians, 0.3% from some other race, non-Hispanic, and around 2.2% of two or more races. Overall, people with Spanish, Latin, and Hispanic origin consisted of around 9.4% of the total population.
A study, conducted at the University of Wisconsin, Milwaukee has lately reasoned that Indianapolis can be counted amongst the most segregated city in the North side of United States (Miller, Vandome and McBrewster 2010).
