Reason for Referral

The client is an 18 year old high school senior who is failing most the classes she has taken. She was recently witnessed of joining an intervention program of intensive outpatients who are facing issues relating to residential placement. The client had helped in trashing the house of the neighbor during a party in August 2008. Even though proof was present in the form of pictures on the client's own Photo Bucket Web page, she denied committing any such act. The pictures showed the client pouring liquid, which was used to wash dishes in to the salt water aquarium. An evaluation of emotional/social difficulties was referred to the client by the Department of Corrections.

Test(s) Administered: WAIS-IV

The WAIS-IV was administered. The WAIS-IV is considered to be a test of intellectual abilities. The age range for this particular test is between ages 16-90 years.

Background Information

Client is high school senior. There are no reports of any other siblings. Client's parents are employed as radiology technicians at the local hospital. She has no history of trauma, family crisis, or discord including divorce or recent moves. Client is healthy, and she has no history of attention problems. She passed all of her classes until age 12.

The client is an 18 year old high school senior who is failing most the classes she has taken. She was recently witnessed of joining an intervention program of intensive outpatients who are facing issues relating to residential placement. The client had helped in trashing the house of the neighbor during a party in August 2008. Even though proof was present in the form of pictures on the client's own Photo Bucket Web page, she denied committing any such act. An evaluation of emotional/social difficulties was referred to the client by the Department of Corrections.

The client has had a diagnosis which highlighted a dysthymic disorder as well as at the age of 15; the client also had an acute care psychiatric placement. This was forced due to the fact that the client threatened to commit suicide just because she thought her boyfriend was cheating on her with another girl. Since then, no other such incidents have been reported. Dr. Alvin C. Monk, who is an MD of Board Certified Psychiatrist, prescribed the regular use of Effexor to the client.

Behavioral Observations

The difficulties and the efforts of the establishment of a relationship with the client were noted during testing. The client has had lot of academic difficulties in the past, which it is facing in the present as well. This had restrained her in expressing her true ability of intellect in this assessment.
