The errors that I can spot from the questionnaire are:
In question no. 2 the limits specified are not precise. For instance it's not clear whether a student who has borrowed 5 books from the library will tick 1-5 interval or 5-10 interval. He can tick either based on what he thinks right and can distort the results in the end. The intervals should have been 1-4, 5-9, 10-14 and so on Once again in question no. 2 a student who has borrowed more than 100 books has no choice to mark. There should have been a choice of more than 100 in the same question.
In question no. 3 there are only 4 purposes specified. There may be other reasons for students to visit the library. For instance issuing a book from the library etc
Question no. 4 is a dichotomous question with only yes or no options. Someone who visited the library for more than one reason and only one purpose was fulfilled and other were not then what is he or she supposed to mark.
Question no. 5 is a ranking question and the filler of the questionnaire is provided with five ranking options. It's not specified whether the rankings are moving in a ascending or descending order i.e. it's not clear whether 1 is the highest satisfaction level or lowest. The rankings should have been on the terms like highly satisfied, satisfied, average, not satisfied etc.
Question no. 7 asks the filler of questionnaire about any Improvements that could be made to the services of the library. There should also be some comments by the filler on how these improvements can be made. Someone who says yes there is a room for improvement must be given a chance to state them
The options in question no. 11 should be yes or no rather than against or not against. It could have been simplified a bit by just writing: “are you against having longer opening hours?”

The suggested questionnaire has some flaws as mentioned above. It does not provide enough clarity to the current users regarding their future needs. It does not address the issues that may face the users of public library and also does not properly address the ways any flaws can be removed and services improved.
