Introduction
This case study is based on analysing the management and monitoring or a client, who is diagnosed with diabetes in primary care setting. The case study will help the reader in identifying and examining the main cause behind diabetes and what are the effective treatments that can help in curing it. This case study is based on a 70 years old man who has been diagnosed with diabetes.
One of the most challenging diseases that United Kingdom is facing is Diabetes that is continuously growing. If we want to judge the growing health calamity then we should look at the number of people dying from diabetes in the country. The main reason behind the growing health crisis due to diabetes is due to lack of its awareness in the general public. We need to raise the awareness of the risks and consequences attached to diabetes through arranging wide range of medical campaigns and workshops by various medical institutions. People are still not aware of how to manage their lifestyle when they are diagnosed with any disease like diabetes. Patients of diabetes need to be very careful about their health and should follow the health guidelines given by their doctor. The role of integrated diabetes care services can play an active role in the management of this highly growing disease (Tupola et.al, 1998, p.695).
Case Study
Mr. RL was working in security firm when he was diagnosed of diabetes. He was working as a HGV driver before joining this firm. Mr. RL's family was found to be suffering from diabetes and his own son was having diabetes and was on insulin. This was very strange that he never had his own diabetic check. This was the main reason he was not diagnosed of diabetes for a long time. His brother and son were on insulin as they were not able to produce enough insulin they were having type 1 DM. Mr. RL never took care of his health and never had any health check up. He was a chain smoker and was smoking about 25 to 30 cigarettes a day. He was a moderate drinker and was having but since he was diagnosed of diabetes, he minimized it use and even stopped smoking at once. He was brought to hospital in critical condition and was partly un-conscious due to health disorders.

The haemoglobin A1C test (HbA1c) is essential for diabetes patient as it gives them strength to fight against diseases. Mr. RL is having haemoglobin more than the normal level that is 6.5% for diabetes patient and for non-diabetic patients it is 3.5% to 5.5%.
