Background

Nursing-Patient relationship is based on the communication level and the interaction for nursing that has been often stated point (Millard, et.al, 2006). Nursing-Patient interaction It is only a way of communication that nurses use in the clinical health care setting that help patient to receive from his/ her disease (Shattell, 2004). This paper aims to discuss the nursing interaction with patient (42 years old James) with having five chronic study disease because of hypertension. Step by step process is discussed to identify the key symptoms of his disease and provide methodology for reducing the issue in James.

Patient Information and Medical History

James is a 42 years old and patient of longstanding. Three years ago James was diagnosed with stage 5 chronic study disease because of accelerated hypertension at his home. James also faces a problem of Hepatitis C and asthma for a long period of time. We have seen James for around 12 months and he is now taking a routine appointment.

Because of his medical problem, his IT consultant is not currently working with him. Since 2005, he has been separated from his wife too due to his rental impairment, he is also seen in a disease of advance chronic kidney and he is also recently decided to have a haemodialysis as therapy of renal replacement.

After the meantime of direct questioning with James, he is reporting that he is tried to the point of wariness and states that his memory has been badly affected recently, he also could not find any interest in his hobbies and also finding a difficulty to enjoy the every day activities for example watching his favourite program on television as well as sharing a good meal with his family members.

On Examination

After having an half an hour chat with Mr. James, he is speaking mostly short sentences and also rarely makes an eye contact. Majority of his replies are also find on the basis of 'yes' and 'no' and he is also frequently the needs of direct questions to get the prompt answers. James also shows of poor self care and hygiene (Larsson et.al, 2007).

To find the problem of depression Nurse is requires asking following questions and trying to find the symptoms that are attached to the problem of James.

Nurse X: During the last month; whether you are felt down, hopeless or depressed?

James: Yes, during the last month, I have been feeling too much bored to my self and not would like to view television. In fact, I also don't prefer to watch my favourite television program on television set. In addition I also don't like my food which usually I like most.
