Abstract

For thousands of women that would come behind her Florence Nightingale set the stage for leadership across the world to lead nursing into the next two centuries. The issue of ineffective patient discharge planning and teaching will be properly addressed by this paper. The greater emphasis on continuity of care and increased healthcare costs are the two major reasons due to which in the emergency department, discharge planning has become a serious issue. With relational to inpatient care, discharge planning is a critical and vital aspect, only a few internal medicine residency programs offer a formal discharge-planning curricula. In order for successful implementation, the discharge planning process must be well planned due to its complexity for which it must be a essential feature within relative toward their aftercare and must be observed like being fundamental toward the concerned path as of access beyond. According to research, the involvement of caretakers and patients in decision making is highly important, and different strategies must be adopted in discharge planning to improve participation of the nurses. In the process of planning of discharge, to identify the opportunities and barriers, nurses contribute significantly to evidence-based reform initiatives and are uniquely positioned. Adversely affecting patients' outcome, the continuum of care is usually broken by the low discharge process. It is often difficult to achieve the complexity of the discharge planning process in a totally effective and efficient manner. With current trends toward more community-based health services, increased health care costs and shorter hospital stays, discharge planning has become increasingly important. However, there is little published research and no monographs devoted to the discharge planning in the current emergency nursing. Over the past two decades, the discharge planning has been widely researched and issues ranging to the outcomes of ineffective and effective discharge planning from the implementation of discharge planning and assessment of patients, is covered with the literature. Effectual preparation of discharging could assist toward enhancement of the excellence of existence intended for the patient and to reduce readmissions to the hospital when properly undertaken that is demonstrated by some more recent studies. The profession, group or an individual in conduct in the broadest sense is guided by the principles that are the ethics. The proper conduct and behavior of nurse is closely scrutinized from the earliest concept of nursing. Some of the specific issues of moral behavior and conduct were addressed by Florence Nightingale. Nursing has evolved into a very complex field over the last hundred years.
