CHAPTER 1: INTRODUCTION

Physicians earn continually every month regardless of what quality of care they offer to their patients (Doran, 2008). As rising healthcare costs continue to garner national attention, pay-for-performance has been one strategy put forth to counteract the increasing costs and to improve the quality and effectiveness of health care (McDonald, White & Marmor, 2009, 74). Various researchers suggest that the patients suffering from the chronic disease, such as diabetes, obesity, and hypertension do not receive quality care. These are the conditions that require continual monitoring (McGlynn, 2003). These are the most crucial factors in terms of harmfully affecting health related quality of life (McGlynn, 2003). This study ensures high quality care, emphasis on driving improvement in performance and variation, in quality, and performance (Doran, 2008).

1.1 Problem Statement

Hypertension, diabetes, and obesity are medical conditions that often require continual monitoring by physicians. The health care system of the United States struggles with the inadequate care coordination for individuals suffering from chronic illnesses (William, 2012). The problem is that all the physicians in the hospital earn the same amount of money regardless of the quality of care provided. Therefore, the Health care delivery system of America is in need of fundamental change (Hicks, 1998). The levels of frustration of both the clinicians and patients are increasing.

Everywhere the patients get affected by the experience of problems regarding quality (Chen, 2010). In the current years, the industry of health care leaders have joined to pay the physicians a large amount of finance based on the differences in the quality, offered of their services that principally determines what they have achieved on the quality performance measure (National Quality Forum, 2003).
